What is the distance between Manciano and the Terme di Saturnia?

From Manciano, allow about 14 to 15 km up to the Saturnia Thermal Baths, i.e. around 15 to 20 minutes by car depending on traffic and the exact place you are heading to. The spa facilities and the waterfalls of Cascate del Mulino are not exactly at the same spot: between one and the other, check your destination carefully in your GPS.

The car is the simplest solution for this trip. Without a car, check the bus schedules before leaving: the connections may be less convenient for reaching the hot springs, and you won't want to discover on site that you have to arrange the return trip differently.
