Near Follonica, start with Scarlino (about 8 km, about fifteen minutes by car), then head for Massa Marittima (about 20 km, around 20 to 25 minutes) if you want to see a larger medieval town. Scarlino sits perched above the gulf: the village is quick to visit, mainly for its alleys and the Rocca. Distances and durations vary depending on your starting point and traffic.
Gavorrano, about 15 km, is worth the detour if mining history interests you; the village is quiet and the visit takes on more meaning with its park and its mining-related sites. For a longer outing, Suvereto is another option to the north, with a compact old town. These are not seaside stops: plan rather half a day to combine two of them at a leisurely pace.
If you prefer to stay on the coast, Castiglione della Pescaia is about 25 km of Follonica: the old village combines easily with a stroll around the port, but in summer traffic and parking can lengthen the outing. For a first excursion, I would choose Scarlino and Massa Marittima, which offer two very different atmospheres.
