In Italy, I would choose Hertz, Avis, Europcar or Sixt, comparing the final price and conditions instead of relying only on the displayed rate. For a stay in Tuscany or the Maremma, choose an agency directly at the airport or train station if you pick up the car on arrival: a cheaper off-site rental company may add a shuttle and waiting time. Rates vary a lot depending on the season and location; in juillet-août, book early and check the total cost, insurance and fees included.
Before paying, pay special attention to the amount of the deductible, the deposit blocked on the card and the rules regarding additional drivers. A credit card in the main driver's name is often the simplest solution; confirm the exact conditions with the agency. At pick-up, photograph the bodywork, the wheels and the windshield, then have any damage noted on the contract. For a first time driving in Italy, a small car is more practical in villages and narrow parking lots. Finally, watch out for ZTL : historic centers are often off-limits to unauthorized cars, and fines can arrive after the trip.
